This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. HSCG27-08-Q-3NT235 is issued as a {Request for Quotation (RFQ)}. The incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 05-14. The applicable NAICS number is 326299 and the small business size standard is 500 employees. This will be awarded as a Firm Fixed Price award. This is a small business set aside procurement. The FOB Destination delivery is to: USCGC FORWARD (WMEC 911) 4000 Coast Guard Blvd, Portsmouth, VA 23703 on or before: 4/20/08. Offers are due at: USCG ISC Portsmouth, VA, by the Close of Business on 3/14/08. Questions may be directed to John Lukondi, telephone 757-483-8555. If cutter is not in port please contact LCDR Tim OBrien of the CGC LEGARE (757)483-8750. Cutters are identical so any measurements or viewing can take on board LEAGARE. The USCGC FORWARD has a requirement to purchase the following: 1. GENERAL REQUIREMENTS: The work consists of providing all labor, materials and equipment as required to remove the current dielectric matting (where previously matting exists), underlay (as required), and install new underlayment and matting in the compartments listed, at this location: USCGC FORWARD (WMEC 911) 4000 Coast Guard Blvd Portsmouth, VA 23703 2. COMMENCEMENT, PROSECUTION, AND COMPLETION OF WORK: Upon acceptance the contractor shall commence work NLT five (5) days after notice to proceed and contract must be completed by 20 April 08. 3. SCOPE OF WORK: a. Notes: 1. Seams in this compartment shall NOT be heat welded. Seams shall be sealed using 4 inch wide 3M electrical insulating tape underneath the seams (i.e. the tape shall not be exposed). 2. These compartments are secure spaces and ships personnel are required to escort contractors at all times they are in the compartment. Due to this requirement, the contractor shall only plan work in these spaces between 0700 and 1500, Monday through Friday. If work is required outside of these times, advanced notice (at least 48 hours) must be given to the ships representative. 3. Dielectric matting and underlayment does not currently exist in these compartments. New dielectric matting shall be laid directly over the existing One-Step epoxy decking. 4. Underlayment does not currently exist in this compartment. New matting shall be laid over the existing deck plates. New matting will NOT be glued to the deck plates, and cove base is not required around the border of the compartment. c. The contractor shall use plastic sheeting to protect all equipment in each compartment, paying particular attention to electrical or electronic equipment. Additionally, the contractor shall use filter media securely fastened over any exhaust ventilation to prevent the spread of dust throughout the ship. 1. All removable interferences shall be removed by ships force prior to work beginning on the respective compartment. Any equipment remaining shall be considered permanent. If in doubt regarding the permanency of the remaining equipment, the contractor shall ask the ships representative prior to beginning work. d. The contractor shall remove all existing matting and underlay and dispose of IAW all federal and local regulations. e. The contractor shall prepare and coat all exposed steel deck surfaces, including permanently installed mounting brackets and vertical bounding surfaces, in accordance with COMDTINST M10360.3, specifically: 1. Power tool clean the all surfaces to bare metal (SSPC-SP11). i. NOTE: Special care shall be taken in the Pilothouse as the deck is aluminum 2. For all applicable decks EXCEPT the Pilothouse: Apply a zinc rich primer approved by the Coast Guard in accordance with COMDTINST M10360.3 and compatible with the underlay system to a minimum dry film thickness (DFT) of 3.0-4.0 mils. 3. For the Pilothouse: Apply a high build epoxy primer approved by the Coast Guard and compatible with the underlay system to a minimum dry film thickness (DFT) of 4.0-5.0 mils. f. The contractor shall install a new underlay system, specifically: 1. Prepare and apply an underlay material conforming to MILSPEC MIL-PRF- 3135, over the entire primed deck surface of Type II, Class 1 2. Fill in all depressions in the deck; level fair welded seams and deck irregularities and level off underlay to a minimum thickness of .500 inch and up to .750 inch on the deck. The depth of the underlay will be adjusted such that the installed electrical matting will be in the same plane as the upper rims of scuttles and deck fittings, so as not to interfere with the operation of these fittings or create a tripping hazard. 3. Sand underlay, if necessary, to provide a smooth finish. g. The contractor shall install new matting, specifically: 1. Marbelized blue matting which is in compliance with the specifications contained in Matting or Sheet, Floor Covering Insulating for High Voltage Application, MIL-DTL-15562 (series). For more detail, refer to http://assist.daps.dla.mil/online/start/ 2. Matting shall be installed so as to minimize the number/total length of seams. 3. Matting shall be secured to the underlay with a latex adhesive conforming to MILSPEC MIL-A-21016. i. NOTE: The exception to this requirement is in the Combat Information Center (CIC), where mating will NOT be glued to the deck plates 4. Immediately after the matting has been cemented to the deck, the contractor shall thoroughly roll a 150-pound sectional roller over the deck covering to facilitate adhesion. 5. Silicone sealer (MIL-A-46106, Type I) shall be used to waterproof all seams against bulkheads, permanent equipment/furniture, piping, or other deck fittings. i. NOTE: No silicone sealer is required in the Combat Information Center (CIC). 6. Seal seams between sheets by heat welding. i. NOTE: In the Combat Information Center (CIC) all seams will be sealed using 3M insulating tape, as required in Section 3.b of this Statement of Work ii. NOTE: All supply ventilation and A/C will be secured while completing all heat welding in order to ensure proper weld quality and prevent cracking due to cold/undercut welds. iii. NOTE: The technician conducting the heat welding shall be well practiced in this technique. 7. Contractor shall install a 4 inch high black vinyl cove base to a maximum height of 4 inches at all bulkhead boundaries with cove base adhesive. i. NOTE: No cove base is required in the Combat Information Center (CIC). 4. SITE VISIT: All bidders are strongly encouraged to visit the site to field verify any/all quantities before submitting bids. Interested parties shall make arrangements for a site visit with LT Michael Cilenti (757)483-8740 or Michael.A.Cilenti (at sign) uscg.mil. If the cutter is not in port please contact LCDR Tim O'Brien of the CGC LEGARE @ 757-483-8750. Their cutter is the exact same as the FORWARD so any measurement or viewings can take place there. 5. DESIGNATED CONTRACTING OFFICERS REPRESENTATIVE: The designated contracting officers representative and Point of Contact for this project is LT Michael Cilenti, (757)483-8740. Inquiries concerning any phase of the specification before or after award shall be made to the above POC. Also any contractor unable to access MILSPECs and COMDTINST M 10360.3 can contact the COTR via e-mail at Michael.A.Cilenti@uscg.mil for a copy of all documentation. 6. CONDITIONS AFFECTING THE WORK: The Offeror should visit the site and take such other steps as may be reasonably necessary to ascertain the nature and location of the work, the general and local conditions which can affect the cost of the work thereof. Failure to do so will not relieve bidders from responsibility for estimating properly the difficulty or cost of successfully performing the work. The government will assume no responsibility for any understanding or misrepresentations concerning conditions made by any of its officers or agents prior to the execution of the contract, unless included in the request for quotes, the specification or related documents. 7. AFTER AWARD OF THE CONTRACT: The contractor must immediately notify the contracting officers representative of his intended start date. The work shall be done in accordance with the specification. 8. ORAL MODIFICATION: No oral statement of any person other than the contracting officer shall in any manner or degree modify or otherwise affect the terms of this contract. 9. WORKMANSHIP: All work shall be accomplished with the best standard practices, and by workmen thoroughly experienced in the required trades. The contractor shall use plastic drop clothes or other suitable means to prevent the spread of dust and debris from the mess deck into equipment, ventilation and into adjacent spaces. 10.The contractor shall provide suitable ventilation for curing of all products. The work shall be supervised by experienced personnel and shall be satisfactory to the Contracting Officer. 11. PERMITS: The contractor shall without additional expense to the government, obtain all appointments, licenses, and permits required for the prosecution of the work. The contractor shall comply with all applicable federal, state and local laws. 12. CLEAN UP: All trash and debris shall be removed from the site. The job site shall be left in a clean and neat manner. 13. WORK OUTSIDE REGULAR HOURS: If the contractor desires to carry on work on Saturday, Sunday, holidays or outside the station?s regular hours (Monday thru Friday 0645 thru 1600), he may submit his request in writing to the Contracting Officers Representative for approval consideration. The contractor shall allow ample time to enable satisfactory arrangements to be made by the government for inspecting the work in progress. 14. UPON COMPLETION OF WORK: Contact the Contracting Officer Representative to arrange for inspection/acceptance of work. 15. IDENTIFICATION OF CONTRACTOR PERSONNEL AND VEHICLES: The contractor shall provide a detailed list of all employees to the COTR 5 days prior to commencing work. The employee list shall contain the employees full name, date of birth, current address, and last 4 of SSN and place of birth. The ISC Portsmouth Security Office will be given a copy of the employee list and a pass will be issued for entry. No underage visitors authorized onsite. Employees and suppliers only, no dependants, minors or anyone else not directly associated with the ongoing work. Each contractor provided vehicle or towed trailer shall show the contractors name so that it is clearly visible on the vehicle. A valid state license plate and safety inspection sticker is required. All vehicles operated on government property shall be maintained in a good state of repair. 16. SAFETY REQUIREMENTS: The contractor shall conform to all state and local safety requirements and those of the Coast Guard ISC Portsmouth. The contractor and his employees shall become familiar with and obey all base regulations including fire, traffic, and security regulations. There will be no smoking in Coast Guard Buildings. No pets or animals of any kind allowed on worksite. The exception being service animals for the visually or hearing impaired. Documentation will be required for the animal to remain on site. Companys quotes should include: (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Price, availability and any discount terms; (7) Remit to address, if different than mailing address; (8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(j) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of Solicitation Amendments; (10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); (11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration; (12) Company Tax Information Number and DUNS Number. All responsible sources may submit a quotation, which if timely received, shall be considered by this agency. The following FAR provisions/clauses apply to this solicitation.
