Transcranial Magnetic Stimulation 1 - Definition and rationale

Definition: rTMS is a non-invasive brain stimulation technique that generates rapidly varying magnetic pulses near the scalp via an electromagnetic coil that This induces an electric field to modulate cortical excitability, thereby treating neuropsychiatric disorders.

Mechanism of action: Stimulates nerve cells in specific brain regions (e.g., left dorsolateral prefrontal cortex, LDPFC) via magnetic fields to modulate their activity.

High frequency (>1 Hz) increases cortical excitability, low frequency (<1 Hz) decreases cortical excitability.

Treatment of depression, PTSD, and other disorders may be related to activation of brain areas with reduced activity due to disease.

FDA Approved Indications: Depression (approved 2008): stimulation of the LDPFC for medically refractory patients.

Chronic Pain (approved 2013): high frequency stimulation of contralateral primary motor cortex (M1).

Obsessive Compulsive Disorder (OCD) (approved 2018): modulation of the orbitofrontal cortex (OFC) and supplementary motor area (SMA).

Smoking cessation aid (approved 2018): high frequency stimulation of LDPFC and superior medial frontal cortex (SMFC).

Migraine: approved by the FDA for prophylactic treatment.

Potential applications in research: Post-traumatic stress disorder (PTSD), Tourette's syndrome, generalized anxiety disorder (GAD), bipolar disorder, Parkinson's disease, functional tremor, epilepsy, and hallucinations (left temporoparietal cortex).
